Integrating Anti-Racist Clinical Skills into Telehealth Simulation

Purpose: To design an anti-racist curriculum that uses telehealth simulations based on the APGO Milestone One cases1 to prepare learners for residency.Reference: 1

 

Background: Postgraduate medical students are expected to begin residency with set of competencies that allow them to practice medicine with limited supervision. This study was aimed at creating a telehealth simulation as part of a clinical skills exam (CSE) to assess learners’ abilities to handle classic Ob-Gyn scenarios with an anti-racist approach.

 

Methods: First, the CSE stations were created based off APGO cases. Eight anti-racist clinical skills (ARCS) were developed and incorporated into the CSE. One CSE station was run as part of a bootcamp for fourth-year medical students matching in Ob-Gyn.

 

Results: Study participants were asked to fill out a post-CSE survey. Most of the students responded positively to the CSE (Figure 1). The students also received numerical grades: the group scored an average of 98.57 +/- 3.63 in professionalism, 97.38 +/- 2.975 in communication, and 65.24 +/- 15.02 in history taking.Link: Figure 1

 

Discussions: This study demonstrates the success of simulation as a tool to assess classic Ob-Gyn scenarios from an anti-racist lens. Due to the positive feedback from students, this study’s participants are currently being expanded to Ob-Gyn residents and clerkship students from other institutions, which will test the generalizability of the case and provide insight into how residents perform on the CSE. This CSE will help future and current Ob-Gyn residents exercise critical thinking, empathy, and an antiracist approach in their clinical care.
