Advancing medical student education through the development and implementation of consensus-based obstetric sonographic objectives

Purpose: To develop consensus-based learning objectives for obstetric sonography in the 2nd/3rd trimesters and implement an online training program for medical students.

 

Background: Obstetric ultrasound plays a key role in quality prenatal care. Medical students are frequently exposed to obstetric sonography during their clerkship, but there are currently no national guidelines that address specific goals or a curriculum.

 

Methods: In the first phase, we developed a set of learning objectives using the modified Delphi approach. A group of approximately 20 expert stakeholders (including leaders in education, clinicians, trainees, and sonographers) reviewed a list of proposed objectives for 2nd/3rd trimester obstetric sonography. An objective was included in the final curriculum if it received the support of at least 75% of the stakeholders with a goal of universal consensus. In the second phase, we designed an online tutorial to achieve the objectives using Camtasia software and a combination of volunteer patients and an ultrasound simulator.

 

Results: Stakeholders reached an agreement to include the following objectives: assessment of fetal presentation, fetal heart rate, amniotic fluid volume, and placental location in addition to general knowledge of antepartum surveillance to include the biophysical profile. Based on these objectives, a 20 minute online tutorial was created. Validation of the objectives and tutorial is forthcoming.

 

Discussions: We established consensus-based objectives in 2nd/3rd trimester obstetric sonography and created an online tutorial to improve the quality of medical student education and to provide a springboard for applying these skills in clinical practice.
