What Did You Learn Today? Assessing How an After-Clinic Knowledge Dive (ACknowledgeD) Affected CREOG Scores
Purpose: To assess whether initiating a peer-to-peer, small group learning initiative in the ambulatory setting, correlates with improvement in CREOG scores.
Background: Resident continuity clinics often lack a coordinated educational curriculum. From February 2019 to February 2020, the “After Clinic Knowledge Dive,” an innovative outpatient educational curriculum, was integrated into resident continuity clinic at Montefiore Medical Center (Bronx, New York). The ‘dive” became an effective, yet informal way, to teach evidence-based medicine in clinic while employing milestone practice-based learning and improvement.
Methods: Residents reflected on their experience, identified a deficit, and then taught their peers, with guidance from attending preceptors. Subject matter was mapped to CREOG exam objectives. Scores were evaluated for 46 participants (blinded, 1-way ANOVA and Kruskal-Wallis).
Results: The “dive” integrated well and the composite office practice, general considerations and specialty objective CREOG scores increased significantly (p < 0.0001). Office scores improved by 2020 for all three upper classes, and overall CREOG scores increased, however the results were not statistically significant.
Discussions: Following the intervention of the “dive,” there was significant improvement in the composite office practice, general considerations and specialty objective CREOG scores. The intervention provides a unique opportunity to address learning gaps in office practice and share that knowledge with peers in a clinical environment.
