Leaning in to the In-Between: Building a Novel Educational Experience into Mid-Clerkship Transition

Purpose: To create a new mid-clerkship educational experience for students as they transition between clinical services.

 

Background: Mid-clerkship feedback, oral presentations, didactic sessions, and transitions between services are standard clerkship components. At the University of Rochester, a transition occurs at the clerkship midpoint, when students switch between two 14-day clinical rotations. In January 2021, we implemented a new educational experience called Transition Day, an innovative, learner-directed, technologically-supported session incorporating mid-clerkship feedback, student presentations, learner self-reflection, and didactic teaching.

 

Methods: Transition Day is a full-day educational experience employing multiple learning modalities. The day involves protected time for: (1) formative feedback meetings between clerkship students and faculty, (2) students to watch pre-recorded topic presentations prepared by their peers and uploaded to a shared media library, (3) students to develop multiple choice questions (MCQs) based upon content in peers’ recorded videos, and (4) an interactive videoconference session with the clerkship director during which students review, submit answers to, and discuss all MCQs.

 

Results: Clerkship evaluations show marked improvement in didactic experience following Transition Day implementation, with 71.3% of students assessing sessions as “very good” or “excellent”, compared to 33.58% prior to the change. Qualitative feedback suggests that students find Transition Day engaging and worthwhile. The clerkship director has also found these sessions satisfying and instructive.

 

Discussions: The newly implemented Transition Day meets student, faculty, and clerkship needs by using the transition between services as an opportunity to give and receive feedback, encourage peer teaching, support self-directed learning, identify knowledge gaps, and complete mandatory clerkship tasks.
