Trauma Informed Care Training in Ob/Gyn Residency: A National Assessment

Purpose: The purpose of this study was to investigate how Ob/Gyn residents are being trained to care for patients with sexual trauma histories, barriers to providing such training and whether there would be interest in trauma-informed care (TIC) training specifically for Ob/Gyns.

Background: One in five American women experience sexual abuse in their lifetime, which pre-disposes a large percentage of our patients to longstanding trauma. Patients report being re-traumatized when undergoing exams during childbirth and routine Ob/Gyn visits, however little is known about whether Ob/Gyn physicians are trained to care for patients who have experienced trauma.

Methods: This was a cross-sectional survey administered to program directors at all US and Canadian Ob/Gyn Residency programs.

Results: Ninety-two out of 275 (33.5%) of programs responded to the survey and were geographically diverse. When asked about whether they provide formal training for the care of patients with sexual trauma histories, 19.6% of programs have training that occurs yearly, 53.3% have had training in the past though not regularly, and 27.1% have never had any training on this topic. Only 25.3% of respondents were satisfied with their current training.  Ninety-three percent of our sample believe that Ob/Gyn residents should be trained in TIC, and 65.5% believe it should be a CREOG educational objective. If a TIC curriculum were developed specifically for Ob/Gyn residents, 92% of programs would be ‘likely’ or ‘extremely likely’ to implement it.

Discussions: Most Ob/Gyn residency programs are providing some training on the care of patients who have experienced trauma, however most are unsatisfied with how this occurs. There is significant interest in incorporating TIC training specifically for Ob/Gyn residents.
