Filling the potholes: Analysis of the OBGYN Standardized Letter of Evaluation

Purpose: To analyze assessment ratings on the OBGYN standardized letter of evaluation (SLOE)

 

Background: The SLOE was introduced to provide a competency-based, standardized evaluation of an applicant. Criterion-based ratings in communication skills (CS), patient care (PC), procedural skills (PS), medical knowledge (MK), professionalism (Pro), practice-based improvement (PBI), and system-based practice (SBP) and areas for improvement are required.

 

Methods: All applications submitted to one academic residency program were reviewed. Demographics including gender, race/ethnivcity, and medical school type (MD, DO, International) were analyzed in relation to SLOE competency ratings. Associations of applicant factors with SLOE competency ratings and the relationships between SLOE ratings and other grading metrics were analyzed.

 

Results:
474 of 916 (52%) applications contained one or more SLOE (total SLOE=485). 111 SLOE (22.9%) had “intern level” ratings across all competencies. No differences in any of the competency ratings were found with regards to gender or race/ethnicity. Significant differences in ratings were found with school type in CS, PS, Pro, and SBP. Correlation was found between top clerkship grade and “intern level” for MK and PC and between top MSPE rating and “intern level” for MK. USMLE Step 1 and Step 2 scores correlated with SLOE ratings for CS, PC, and MK.  

 

Discussions:
Gender and race do not appear to influence SLOE ratings. Some internal consistency was shown in the SLOE supporting its utility, however its role in holistic and standardized review of residency applicants is yet to be determined.
