Entrust or Bust: Eliminating Bias in Assessments on the Road to Clinical Competency

Objective: Core Entrustable Professional Activities (EPAs) define a set of observable activities all graduating medical students are expected to perform without direct supervision. Medical schools have begun adopting EPAs for student assessment, and specialty-specific EPAs linked to OBGYN Milestones are under development.  Specialty- specific EPAs can facilitate the transition from undergraduate to graduate medical education.However, faculty bias in defining expectations of an “entrustable student,” may jeopardize equity in assessment. Developing assessment tools and frame-of-reference training for faculty is essential to ensure grading equity.

 

Participants will develop standard definitions of entrustability for OBGYN-specific EPAs through active learning techniques and group discussion. Participants will explore concepts of equity in grading and hear learners’ perspectives on bias and equity in assessment.

 

Methods:  We will survey participants for bias mitigation strategies and for efficacy of video training materials in decreasing bias by establishing standardized levels of achievement in the EPA framework.

 

Results: Students from traditionally minority or marginalized populations commonly receive lower grades and fewer awards and honors in medical school, which may stem from bias in grading rubrics. Our presentation focuses on developing Specialty-specific EPA assessment frameworks and providing frame-of-reference training to identify and address the potential for implicit bias in performance assessment.

 

Conclusion/ Discussion: With an objective description set in place, it is our hope to eliminate potential subjectivity that may arise in student competency assessment, and to increase equity in the face of ongoing biases in medical education.
