A Video Curriculum to Improve Students’ Confidence and Willingness to Discuss Pain and Substance Use Topics in OB/GYN

Purpose: To evaluate the effect of a didactic concept video on clerkship students’ comfort with addressing OB/GYN pain medicine and substance use topics.

Background: Despite the prevalence of pain as a presenting chief complaint or a result of our treatment, many medical students have limited exposure to a formal pain medicine curriculum. Prior to implementation, clerkship students lacked dedicated content to pain medicine topics as related to OB/GYN patient care.

Methods: This was a prospective cohort study of OB/GYN clerkship students. A 10-minute concept video on OB/GYN-specific pain medicine topics was developed. Students were asked via a pre- and post-session surveys to rate their self-perceived comfort in discussing pain medicine topics with patients. Students’ comfort was evaluated using a 5-point a Likert scale. Differences before and after were assessed using paired sample t-test.

Results: Seventy-five students (78%) took part in this study, and 64% reported increased comfort discussing etiologies of pain with OB/GYN patients after video viewing (mean pre/post-test scores 2.83 to 3.65, p < 0.001). Seventy-two percent reported increased comfort discussing pain management strategies after viewing the video (mean pre/post-test scores 2.72 to 3.69, p < 0.001). Eighty-eight percent of students were more likely to discuss pain with their patients after viewing the video.

Discussions: Implementation of a pain medicine concept video for OB/GYN clerkship studies improves students’ self-perceived confidence and willingness to discuss pain with their patients. Further work is ongoing to evaluate student efficacy in conducting these discussions while on their OB/GYN clerkship.
