Virtual OSCE as Student Assessment Tool
Purpose: To use virtual patient simulation encounter to evaluate students’ clinical abilities and provide meaningful feedback. Clinical abilities included taking a history, choice of physical exam, interview skills, and development of management plans for common OB/GYN complaints.
Background: Even before COVID19, our OB/GYN clerkship struggled to obtain quality clinical evaluations of our students. When we converted to a virtual clerkship due to COVID19, we developed a virtual patient OSCE that could allow us to assess student clinical skills and identify knowledge and application gaps.
Methods: Each student had a 15 minute virtual meeting with the clerkship directors. One clerkship director would role play a patient. Each student was given a chief complaint and instructed to obtain a history from the “patient”. Next, the student would ask for components of the exam and labs or imaging and we would provide findings and results. Lastly, students would present their assessment and plan. All 3 clerkship directors completed an evaluation for each student.
Results: 58 students completed the virtual OSCE. The average number of evaluations these students received was 5.48, compared to 4 prior to the OSCE (p< 0.01). Prior to the OSCE 1-3 students each term received no written feedback. After the OSCE, every student received written feedback.
Discussions: Our virtual OSCE was so beneficial to our student evaluation and education process that we decided to incorporate it into our traditional clerkship even as we begin to return to in-person clinical rotations. This OSCE can be replicated in other programs to benefit their students.
