A Comprehensive, Longitudinal Approach To Teaching Quality Improvement Through Obstetrics & Gynecology Residency

Purpose: Design, implement, and evaluate a longitudinal obstetrics and gynecology residency patient safety (PS)/quality improvement (QI) curriculum.

 

Background: Despite the ACGME requiring residents to learn practical knowledge around PS/QI, the 2021 CLER National Report of Findings found that many programs teach only components of PS/QI through single, event-based activities. Data are limited on longitudinal PS/QI curricula.

 

Methods: PGY-1 residents completed modules and didactics. PGY-2 residents completed a Root Cause Analysis and Action (RCA2) on a safety event. PGY-3 and 4 residents completed an independent QI project. Residents also presented PS cases at Departmental Morbidity and Mortality conferences. Effectiveness of the program was assessed via the Quality Improvement Knowledge Tool (QIKAT-R) and rubric-based assessments of the RCA2 and QI projects. Assessments were then used to evaluate achievement of the ACGME PS/QI milestones. Reaction data were also collected.

 

Results: Six residents have completed the entire four-year program, with 24 residents currently participating. Foundational knowledge, assessed via the QIKAT-R, improved significantly pre- and post-course (n=10, p 0.0001). Self-efficacy in root cause analysis domains improved significantly via retrospective pre-post test (n=12, p< 0.05). All graduating residents met the level 4 PS/QI milestones. Program sessions were rated good to very good in quality.

 

Discussions: Implementing a progressive PS/QI curriculum helped residents build knowledge, confidence, and tangible skills to be actively involved in PS/QI efforts in residency and beyond.
