The Angry Patient: Objectively Evaluating Obstetrics and Gynecology Residents’ Perceptions of Communication Competency and Professionalism

Objective: To evaluate the benefit of using an Objective Structured Clinical Exam (OSCE) , post encounter self-evaluation, communication skills training intervention and video self re-evaluation of Obstetrics and Gynecology residents’ perceptions of communication competency and professionalism when dealing with an angry patient.
Study Design: Performance was assessed by two faculty, a standardized patient and the resident following an OSCE using a modified version of the American Board of Internal Medicine’s Patient Assessment (ABIMPA) survey. Four weeks later residents participated in training on de-escalating an angry patient in a professional manner, reviewed their videotaped encounters and completed the ABIMPA.
Results: Results indicate that PGY-4 and PGY1 residents overestimated their skills while PGY-3 and PGY-2 residents underestimated their skills on the post-encounter as compared to video self re-evaluation.
Conclusions: The OSCE, skills training and re-evaluation of video-taped encounters are useful pedagogical tools for adjusting self-perception of resident communication skills and professionalism.
