Enhancing OBGYN Resident Preoperative Preparation via a Mobile App providing Just-In-Time Training for the Operating Room (JITTOR)

 Precis: We created and tested an educational mobile-based app for Obstetrics and Gynecology residents that provides just-in-time training prior to the operating room by detailing the steps of a vaginal hysterectomy.
 
 Purpose: To develop a mobile app providing “Just-in-Time Training for the Operating Room” (JITTOR) and to assess residents’ technical skills and satisfaction using the app for vaginal hysterectomy simulation.
 
 Background: Just-in-Time Training (JITT) allows learners to acquire relevant knowledge immediately prior to when the knowledge is needed. This reduces cognitive load and increases efficiency of information processing. There is limited data on the application of JITT in Obstetrics and Gynecology. Therefore, we developed an app utilizing JITT and assessed residents’ performances during a surgical simulation.
 
 Methods: Third and fourth-year residents at five institutions prepared for a vaginal hysterectomy simulation with traditional methods. Half were randomized to have access to the JITTOR app. Performance was evaluated by a blinded investigator using the Vaginal Surgical Skills Index (VSSI). The primary outcome was resident performance in the simulation and secondary outcomes were resident satisfaction and confidence using the app.
 
 Results: Forty-one residents participated. There was no statistically significant difference in performance by VSSI score between those who had access to the JITTOR app and those who did not.
 
 Satisfaction surveys revealed 79% would like additional surgical checklists on the app, 75% felt the app offered added value compared to other methods, 89% would use the app in the future, and 95% found the app easy to use. Regarding confidence surveys, 89% felt the app helped them anticipate steps, prepared them for surgery, and improved their efficiency.
 
 Discussion: While median VSSI scores did not differ between groups, residents expressed high satisfaction and increased confidence during simulation after use of the app.
