The Effect of Formalized Resident Education on Patient Counseling for Interval Sterilization

 Précis: After a formalized education reviewing patient counseling on interval laparoscopic sterilization, knowledge of the subject increased among OB/GYN interns.

 Purpose: To develop and implement a formalized resident education and best practices checklist on patient counseling for interval sterilization and observe its effect on outpatient sterilization consultations in a high-volume resident gynecology clinic.

 Background: At our institution, residents offer patients seeking interval laparoscopic sterilization a choice of two techniques. Laparoscopic salpingectomy is superior to tubal occlusion with clips as it is more effective at preventing subsequent pregnancy and reduces lifetime risk of epithelial ovarian cancer. Resident reported patient counseling content widely varied regarding the risks and benefits of tubal occlusion with spring clips and bilateral salpingectomy. There was no uniform patient counseling checklist or education on evidence-based practices.

 Methods: Resident education and patient counseling checklist for interval sterilization was developed and administered during OB/GYN intern orientation. 13-item pre and post assessment of resident knowledge was anonymously obtained. 

 Results:  Resident assessment completion rate was 100% (n=18).  Mean resident score prior to the educational interventional was 56%, ranging from 38% - 77%.  After the educational intervention mean score was 90%, ranging from 77%-100%.  The average change in test score was +32.48% with the median change in score at +27.78%.

 Discussion: Directed resident education on evidence-based interval sterilization guidelines improves resident knowledge on the topic.  Prospective chart review is underway to determine if this educational intervention translates to resident counseling and affects trends in patient sterilization.
