Impact of Surgical Video Coaching on Resident Surgical Skills: A Cluster Trial

 Purpose:To evaluate if surgical video based coaching intervention can enhance a gynecology resident’s technical skills measured by a global rating scale. Secondar ily, the satisfaction of completing this feedback method was evaluated.
 
 Background: Surgery in OBGYN residency has been taught through the “see one, do one, teach one\" method. Surgical training experience has declined due to non-invasive treatments, patient safety concerns, work hour restrictions, and growth of subspeciality fellowships. Thus, alternative modes are required  to supplement and enhance teaching.  Surgical coaching, a partnership between trainee and teacher that allows for skill refinement through goal-setting and reflection, can be utilized to enhance surgical education.
 
 Methods:  PGY1/PGY2 gynecology residents were randomized to receive or not receive video feedback coaching in addition to traditional surgical teaching for hysteroscopy during their benign gynecology rotation.  The coaching cohort received interval coaching and surgical skills reassessment whereas the traditional group were offered coaching following their rotation. Residents were assessed using both a hysteroscopy checklist and OSATS assessment. 
 
 Results: Three of six cohorts received the coaching feedback intervention. No videos were obtained from the PGY1 in group 1 to give feedback. The PGY2 and PGY1/2 in in group 1 and 3 respectively had no follow-up cases for reassessment.  In those that completed the pre and post videos, coaching showed more completed tasks in hysteroscopic myomectomies.
 
 Discussion: Although, currently, there has been no reassessment to compare for refinement of skills following surgical feedback coaching, the intervention was appreciated by learners in respect to more directive feedback on hysteroscopy skills.
