Remote Clinical Learning in the OB/GYN Clerkship: An Innovative Strategy During COVID-19

Purpose: To describe an innovative approach to providing clinical experience for third year medical students during a pandemic.

Background: Due to the COVID-19 pandemic, students were abruptly excluded from UC San Diego clinical settings in March 2020. In response, a novel virtual curriculum was developed to replace the traditional OB/GYN clerkship rotation.

Methods: Students were assigned weekly to one of three remote clinical opportunities. First were daily obstetrics board rounds with faculty using real-time fetal heart rate tracings and patient records to discuss care plans with students. Second were daily gynecology round where students rounded electronically on a patient and discussed key learning points with faculty. Third was a 1:1 remote clinic assignment with a variety of faculty approaches to incorporate students in the patient encounter. An anonymous survey was completed after the rotation.

Results: Fourteen of the 17 students on the rotation completed the survey. Most students (11) strongly agreed that the obstetrics rounds were valuable, while eight students strongly agreed that gynecology rounds were valuable. Eleven students felt that telehealth clinic was valuable. 94% of the students felt prepared for the final year of medical school, and 100% felt prepared for the NBME shelf exam. Mean shelf exam score was higher (84.5) than the pre-pandemic mean shelf score (80.6) with 100% pass rate.

Discussions: In this pilot study, students reported all remote clinical opportunities were valuable. This novel remote curriculum was sufficient to help students prepare for their NBME shelf exam and fourth year.
